Call the freight company phone number on the top of this sheet, telling them you want a prepaid pick up, billed to us. The top halves are for 1 shipping label and copies for the driver. He will give you one back with a tracking sticker as a receipt. When the truck shows up, hand two copies to the driver, who will give you one back with a tracking sticker on it as a receipt. NOTE TO DRIVER: Remanufactured.com does not authorize or pay for liftgate or residential delivery charges except specifically and in writing. Instead, please request reasonable payment at the location where additional services are rendered.
